Overview

In most sports, athletes have 1-2 practices per week. Depending on the sport and athlete, there are 1-3 competitions per season. The major competition for most of the sports is the State Games.

This competition is typically at the end of the season, when athletes show off all of their hard work and progress made over the course of the season. An athlete must compete at a local qualifying meet to be eligible for State Games. Athletes are not required to enroll in their sport's competitions, they can choose to participate only in team practices.

Athlete Medical Form [download form]

The Health History are the first three pages to be completed by the athlete, parent or guardian prior to the exam. The health history is important for two reasons: (a) to have medical information on hand during training and competition, in case of a medical emergency; and (b) to ensure that the physician is informed of the athlete's health history when performing the exam.

The Medical Exam is the fourth page and is completed by a licensed health professional who is qualified to conduct physical exams and prescribe medications. The medical exam page indicates if the athlete has been cleared by a health professional to participate in Special Olympics sports. If an athlete requires further examination due to a concerning health issue before clearance can be determined, a referral form is available on the fifth page. Customization of this form is not permitted.

The sixth form is the Athlete Release Form, which is required to be signed in order to participate in sports.

The seventh page is the Athlete Likeness Release form, which is optional, and provides the ability for sponsors and partners to use photos, videos and stories of our athletes to show the impact of support.
